We have interviewed a company that provides satisfaction surveys and reports. The company is Service Trac located in Arizona. They have been in business approximately 20 years and service Providers throughout the country; thereby, allowing bench marking regionally and nationally.
Service Trac has surveys for SNF, AL and Medical Day that are individually appropriate for the patient/resident and family members. (Please see the attached sample surveys) Note that at the beginning of each survey there are questions that are consistent with the AHCA quality initiatives. The responses to those questions are automatically submitted and tracked. They also offer surveys in at least 6 different languages which can be completed electronically, or in paper format.
A special feature which is most appropriate for subacute patients is to provide the survey directly on a tablet which the patient can complete prior to leaving the building. We have the option to also modify the survey to our preference.
Purpose: To have a uniform and professional survey process that represents all the Members of CARE. Each Provider will be able to compare their performance against other Providers in their region and CARE will be able to represent all Providers to payers, etc. Additionally, it demonstrates to your residents and families that you are concerned enough about your services that you have an independent organization collecting satisfaction criteria. This will also meet CMS standards.
